Can you train yourself to concentrate for 4.5 hours?
Yes, and the way to do it is to practise in short blocks and make them longer. Which sounds obvious until you notice that most advice about exam stamina tells you to do the opposite: sit full-length papers, repeatedly, from the start.
That advice is not wrong so much as badly sequenced. If you cannot hold accuracy for 90 minutes, a 4.5-hour paper does not train you. It exhausts you, produces a bad score, and teaches you mainly that long papers feel terrible.
What the research actually says
You have probably heard that people can only concentrate for ten, fifteen, or twenty minutes. It is one of the most repeated claims in education and it does not hold up. Wilson and Korn went back through the studies it rests on and found little support for it. There is no fixed attention span, and anyone quoting you a number for it is repeating folklore.
What is solidly established is something more useful to you.
Sustained attention degrades with time on task. Mackworth demonstrated it in 1948, watching people monitor a clock face for two hours: detection dropped by 10 to 15 percent within the first half hour and kept declining after that. That finding has survived nearly eighty years of attempts to explain it away. It is called the vigilance decrement, and it is the single most relevant piece of psychology to a 4.5-hour multiple-choice exam.
Note what it says. Not that you stop concentrating after twenty minutes, but that your accuracy on a demanding task falls measurably the longer you stay on it. That matches what candidates report and what we see in the timing data: not a cliff, a slope.
And it responds to training. In a randomised trial, Mrazek and colleagues found that two weeks of focus training reduced mind-wandering and improved GRE reading comprehension and working memory, with the largest gains among the people most prone to distraction in the first place. Two weeks. The people who were worst at it improved most.
So: the decline is real, it is not a fixed personal limit, and it moves.
How to actually build it
Progressive overload, which is how every other endurance capacity is trained. You do not prepare for a marathon by running marathons.
The unit we use is a 25-question subject quiz, which is roughly the shortest block that still demands sustained work. Then:
Start with two quizzes back to back. No break between them. Two is enough to be more than a study session and short enough that you will actually finish.
Hold there until your accuracy is steady. This is the part people skip. The point is not to survive two, it is for the second quiz to score like the first. If your second is consistently worse, you are not ready to add a third. Adding one anyway just makes the fall-off longer.
Then add a third. Then a fourth. Same rule each time. The last block should score like the first before you extend again.
Add a full mock at the weekend, once four back to back is comfortable, when you have the uninterrupted hours a real paper needs.
The thing you are watching for is not your total score. It is whether your accuracy holds from the first block to the last. That is the trained capacity, and it is measurable, which is the whole reason to structure it this way. It is also why our reports time every answer and chart accuracy across the paper: a total tells you how you did, and the curve tells you whether the training is working.

Two things that help more than they should
Do the long sessions at the time of day you will actually write. Endurance is partly circadian. A 4.5-hour session that ends at midnight is not preparation for one that ends mid-afternoon.
Do not rescue a session with a break. The instinct at the 90-minute mark is to get up, make coffee, and come back. On exam day you will not, so on practice day the break is training you for a paper you are not sitting. Push through the dip. Pushing through the dip is the skill.
